Roma had already made it known that current head coach Paulo Fonseca will leave at the end of the season.
In a statement, Mourinho said: “Thank you to the Friedkin family for choosing me to lead this great club and to be part of their vision.
“After meetings with the ownership and Tiago Pinto, I immediately understood the full extent of their ambitions for AS Roma.
"It is the same ambition and drive that has always motivated me and together we want to build a winning project over the upcoming years.
“The incredible passion of the Roma fans convinced me to accept the job and I cannot wait to start next season.
“In the meantime, I wish Paulo Fonseca all the best and I hope the media appreciate that I will only speak further in due course. Daje Roma!”
Mourinho back in Serie A
Mourinho was not unemployed for very long having been sacked by Tottenham only a fortnight ago.
The Portuguese will also keep busy on the radio station Talksport during the European Championships this summer, prior to taking the gig in Rome.
It will be Mourinho's second spell in Serie A having previously taken charge of Inter who he guided to a historic treble including the 2010 Champions League title.
Mourinho will have work on his hands as Roma currently sit seventh in the Serie A table 12 points behind the Europa League places.
The trophy drought on the Tiber has been ongoing since 2008 with Mourinho the eleventh head coach to take charge since their last Coppa Italia win under Luciano Spaletti.
